Financial Services and Insurance

All patients are ultimately responsible for their own bill. You are responsible for knowing your insurance benefits and limitations. Patients who have healthcare coverage are responsible for providing us with complete and up-to-date information regarding their insurance coverage. Please be sure to bring all insurance cards with you to your appointments.
We are participating providers for Medicare and a wide range of insurance companies and plans.  You can start the process by checking with your insurance company to see if you have covered benefits for the specific procedure you wish to have done.  We will file your insurance claim for you for those plans with which we have an agreement. We will collect any required co-payment at the time of service.

For your convenience, we accept Master Card, Visa, Discover, cash and check.

For our refractive and laser vision patients, we recommend CareCredit, and we are happy to help you with the application process.   

Our billing department hours are Monday through Friday, 7:30 a.m. till 4:30 p.m. Eastern Standard time, and we can be reached at 269-428-3300.

Routine Eye Examinations:

At this time, we do not participate with any Vision Plans.  If you have "vision coverage" and are coming in for a routine eye examination with the absence of any medical complaints, please check with your insurance carrier to see if you have vision coverage under your medical benefits package.

Patient Statements:

In an attempt to control costs, we ask that your portion of our fees, including any co-payment and deductible, be paid at the time the service is rendered.  If your insurance carrier deems a procedure or visit to be "not covered," you will be responsible for the complete charge upon receipt of our billing statement.

No Health Insurance Coverage:

Patients without health insurance are expected to pay their charges in full at the time services are rendered. If this is not possible, we will work with you in setting up a payment plan that will work for both of us.  In the event that we create a payment plan for you, we require that 50% of your fees be paid at the time of  service.
